Abstract

Avulsion is the complete displacement of the dental unit (DU) out of the alveolus, caused by trauma, on a high-resilience support structure, causing the tearing of the fibers of the periodontal ligament (PLD), which must be reimplanted immediately. Unfortunately, this does not happen in most cases. The factors like; storage and transport medium, time elapsed between avulsion and replantation, degree of root maturation, root surface treatment, splinting, patient immune response, and operator skill influence the success of replantation. This case report analyzes the management of the upper right central incisor in an 8-year-old patient who suffered an avulsion DU 11, preserved in water and replanted late (42 hours). An inflammatory external root resorption (IERR) was presented as a complication, due to poor prognosis and previous conditions. However, the goal is to restore, at least temporarily, esthetics and function while maintaining the contour, width, and height of the alveolar bone. The tooth can be extracted, if necessary, after a prompt interdisciplinary evaluation.
